Please keep in mind the following deadlines for 2017-2018 financial aid and scholarship applications.
FACTS Financial Aid Application:
Families who want to be considered for financial aid from Holy Name and the Diocese of Cleveland must fill out the FACTS online financial aid application. If you previously received financial aid from Holy Name, it will not be renewed unless you complete the FACTS financial aid form.
To complete the online application form, visit https://online.factsmgt.com/signin/3X2NP. Please complete the FACTS application no later than TOMORROW, Wednesday, March 15. If you have any questions, contact Finance Director Mrs. Bernie Kowal at 440-886-0300 ext. 124, or at bkowal@holynamehs.com.
If you have additional children who attend other Catholic schools in the Diocese of Cleveland, please check with those schools to see which financial aid application provider they are using. Some Catholic schools are now using SMART, thus requiring you to fill out both the FACTS and SMART applications. Holy Name does not require families to fill out the SMART application.
Holy Name-Specific Scholarship Applications:
Completed scholarship applications must be returned to Holy Name by Friday, March 24. If your student still needs scholarship forms, they are available online at www.holynamehs.com/holy-name-high-school-tuition/. The Holy Name Advancement Office also has copies available. Cleveland Scholarship Program:
If you are a city of Cleveland resident, you can apply for a $5,700 tuition voucher. Families are then responsible for paying the remainder of the tuition balance.
If you currently are a Cleveland Scholarship Program recipient, you must fill out the 2017-2018 Renewal Form, found at https://www.holynamehs.com/wp-content/uploads/2017/01/2017_2018_CSPRenewalForm.pdf. After filling out this form, attach a copy of a current utility bill and a copy of your student’s birth certificate, and return it to Holy Name.
If you live in the city of Cleveland, but are not currently utilizing the Cleveland Scholarship Program, we encourage you to apply. You must fill out the 2017-2018 Request Form, found at https://www.holynamehs.com/wp-content/uploads/2017/01/2017_2018_CSPRequestForm-1.pdf. After it is completed, send the form, along with a copy of a current utility bill and a copy of your student’s birth certificate, to Holy Name.
The application deadline for all Cleveland Scholarship materials to be sent into Holy Name High School is Monday, April 24, 2017. All materials must then be sent from Holy Name to the Ohio Department of Education by Friday, April 28, 2017.
